You wake up inside the world’s largest hospital. Not a hospital building. A hospital island. Far from civilization, surrounded by black water and permanent fog, Mercy Island was once advertised as the future of medicine: a remote medical paradise where the greatest doctors on Earth could cure anything. Every massive building on the island is dedicated to a different medical specialty. Dentistry. Plastic surgery. Cardiology. Neurology. Psychiatry. Pediatrics. Oncology. Emergency medicine. Dermatology. Ophthalmology. Orthopedics. Etc. Each department has its own tower, research wing, surgical theaters, patient wards, and world-renowned head physician. But something poisoned the air. A strange airborne compound now drifts through Mercy Island’s vents, fog banks, sealed wards, and hospital corridors. The staff call it the Miasma Spray. It does not simply make people violent. It twists obsession into identity. The greatest dentist in the world became the Tooth Fairy, a deformed head doctor with jagged sharpened teeth who steals teeth from patients, staff, and anyone unlucky enough to enter his building. The most handsome plastic surgeon alive became obsessed with perfection, beauty, and Aphrodite. Now his face is cut, masked, reshaped, and ruined by his own impossible standards. Every specialty has a corrupted leader. Every leader has followers. Nurses still make rounds. Patients still wait for treatment. Orderlies still wheel beds through the halls. Residents still obey their attending physicians. Some fight because the Miasma Spray has rewritten their minds. Some fight because they are terrified. Some fight because they believe their doctor is the only one who can cure them. And some are still sane enough to whisper warnings before the department doors lock behind you.
